Thank you for choosing an Evolution Healthcare Hospital to provide you with the care you require. We are committed to providing continued excellence in the health and wellbeing of people in our care, and we truly value your feedback.

If you wish to make a suggestion, or raise concerns about any aspect of our service or facilities, you can do this by:

  • Speaking to your nurse, a team leader, our general manager, or your specialist
  • Or filling out the feedback form below

If you wish to make a complaint you will find a complaints leaflet in your room. Seven to ten days after your hospital discharge, you will receive an email inviting you to complete an online patient experience survey.

Your Rights and Responsibilities

We are committed to respecting your privacy and treating you with dignity. If you feel that this has not occurred then please speak to the nurse manager or alternatively you can contact us.

Advocacy (independent support)

Access to the Health and Disability Consumer Advocacy Service, is a free confidential service, and is available by calling 0800 555 050, or emailing advocacy@hdc.org.nz. This service will advise on the NZ Code of Health & Disability Service Consumers’ Rights and on making a complaint.
